Pursuant to the requirement of the Personal Data Protection Act 2010, we are issuing this notice to all our valued customer/prospective customer.
During the course of dealings between you and Pahang Pharmacy Sdn.Bhd.;
You will be requested to provide or has provided to us your personal data to be collected, used and processed for purposes in respect of our products and services.
Your personal data will be/is being collected , used and/or processed by Pahang Pharmacy, amongst others, the following purposes :
a) For registration of a user account with Pahang Pharmacy,
b) To send you invites to Pahang Pharmacy’s, promotional materials, advertisements for marketing purposes; for administrative purposes;
c) For assessment and analysis of the market, Pahang Pharmacy’s customers and/or potential customers in relation to Pahang Pharmacy’s products and services;
d) To monitor, review and improve Pahang Pharmacy’s product, services and collection aging processes.
e) To be used for internal record keeping;
f) For info to update credit control and accounts reconciliation purposes;
g) To alert goods and services similar to those that you have purchased and payment disputes (if any);
h) To comply with any legal or regulatory requirements relating to our provision of services and products and to make disclosure under the requirement of any applicable law, regulation, direction, court order, by-law, guideline, circular, code applicable to Pahang Pharmacy, and/or
i) Update payment received by customer and/or post customer loyalty programmes;
j) Send you statement of accounts, invoices, delivery orders, purchase orders, credit notes, debit notes, e-mail notification/attachment, receipt, audit confirmation, reports, goods return notes, any other material relevant for order, delivery, collection and accounts purpose (where applicable)
k) Such other purposes relevant to the products and services provided by Pahang Pharmacy.
The personal data includes but not limited to the following information :
a) Customer/company name;
b) Customer bank name;
c) Customer bank account no;
d) Business registration no;
e) Payment e-mail address (for payment made notification);
f) New i/c no ;
g) Old i/c no;
h) Customer bank branch;
i) Customer bank address;
j) Contact person;
k) Phone no & fax no;
l) Company mailing address;
m) Bank account type;
n) Bank account code;
o) Bank swift code;
p) Customer code;
q) Company delivery address;
r) Year of business establishment;
s) Type of organization;
t) Authorized capital;
u) Name of property, partner, director, i/c no and status of owner;
v) Customer category;
w) Poison License, APC, Form 12, 9, 24, 49, letter of guarantee, business registration form-CCM;
x) Latest 2 years statutory account;
y) I/C photocopy;
z) Cheque no/serial no;Lei
The failure to obtain your personal data will lead to the following :
a) Pahang Pharmacy will not be able to provide you with updates in relation to Pahang Pharmacy’s products and/or services and/or launches including updating our accounting records;
b) Pahang Pharmacy will not be able to provide you with any notice, services and/or products in which you have requested; and
c) Affect the ability of the parties to enter into necessary agreements and documentations in respect of the sale of Pahang Pharmacy’s products and/or services and/or any related transaction.
Pahang Pharmacy will keep your personal data confidential but you hereby consent and authorize Pahang Pharmacy to disclose your personal data to the following categories of parties;
a) Any person to whom Pahang Pharmacy is required to do so by law or in response to a competent or government agency;
b) Any related, affiliated and subsidiaries of Pahang Pharmacy;
c) Pahang Pharmacy’s business partners in relation to providing you with Pahang Pharmacy’s products and services;
d) Government agencies, statutory authorities and industry regulators;
e) Pahang Pharmacy’s auditors, consultant, accountants, lawyers or other financial or professional advisers;
f) Pahang Pharmacy’s contractors, sub-contractors or third party service or product providers as may be deemed necessary;
g) Third party companies/entities in respect of the products services that customers have purchased ; and/or
h) Pahang Pharmacy’s appointed service providers in relation to Pahang Pharmacy’s loyalty programs, for the purpose of delivery of gift redemption and services.
We shall keep and process your data in a secure manner. We endeavour, where practicable, to implement the appropriate administrative and security safeguards and procedure in accordance with the applicable laws and regulations to prevent the unauthorized or unlawful processing of your personal data and the accidental loss or destruction of, or damage to, your personal data.
Where Pahang Pharmacy considers it necessary in relation to the provision of products and services to you to transfer your personal data to another member of Pahang Pharmacy or third party service or product providers within or outside the country in which Pahang Pharmacy is established, under conditions of confidentiality and similar levels of security safeguards.
It is your right as a data subject to have access and to correct your personal data in which you have provided to Pahang Pharmacy and in this respect, you may :
a) Check if we hold or use your personal data and request access to your personal data;
b) Request for correction of any of your personal data which is inaccurate, incomplete or not up to date;
c) Request for explanation on the policies and procedures in respect of your personal data provided to Pahang Pharmacy;
d) Request Pahang Pharmacy to stop processing your personal data for marketing purposes;
e) Request Pahang Pharmacy to limit the process of your personal data, including personal data limiting to the person who may be identified from your personal data;
f) Inform Pahang Pharmacy on your withdrawal (in full or in part) of your consent given previously to Pahang Pharmacy subject to any applicable legal restrictions, contractual conditions and a reasonable duration of time for the withdrawal of consent to be affected.
